During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated on 10/22/25 and completed on 10/22/25, as a part of an Intensive Plan of Correction (IPOC). The Department requested that the provider complete an IPOC as result of the last inspection related to administration, physical plant, staffing and supervision, special provisions and emergencies, and background checks.

The on-site inspection began at 10:00am and ended at 11:40am. The inspector reviewed compliance in the areas listed above. The inspector reviewed four children's records and four staff records on-site. This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 11/20/25. A POC submitted after this date will not appear on the public website.

(Repeat Violation) The center is required to obtain background checks from any state in which the individual has resided in the preceding five years prior to employment.

The out-of-state criminal history search, the out-of-state sex offender registry search, and the out-of-state child abuse and neglect registry search for Staff #4, employed for three days, were requested one day after the date of employment.
Plan of Correction: Per the Center: "All future employees will obtain all necessary document before their first day."

(Repeat Violation) Hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

In the School Age classroom, a bottle of cleaning solution was observed in an unlocked cabinet in the bathroom used by children.
Plan of Correction: Per the Center: "Corrected during inspection. Management will do daily checks to ensure hazardous substances are locked and put away."
