(Repeat Violation) The center is required to obtain background checks from any state in which the individual has resided in the preceding five years prior to employment.

The out-of-state child abuse and neglect registry search for Staff #1, employed for two days, had not been requested.
Plan of Correction: Per the Center: "All staff will have registry search completed prior to first day of employment."
Standard 8VAC20-780-160-A
(Repeat Violation) Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

The TB for Staff #1 was dated two months prior to the date of employment. Staff #1 has been employed for two days.
Plan of Correction: Per the Center: "All staff will have necessary paperwork/documentation completed prior to first day of employment."

A tuberculosis (TB) screening is required every two years from the last screening.

Staff #2's updated TB screening was due over 2 months ago. The updated screening has not yet been completed.
Plan of Correction: Per the Center: "Staff member has since submitted TB screening."

(Repeat Violation) When an authorization for medication expires, the facility shall notify parents that the medication must be picked up within 14 days of the authorization expiring or that a new authorization should be completed.

Medication for Child #6 was onsite, but the medication authorization had expired over 12 months ago. The medication had not been administered past the expiration of the authorization. The authorization had not been renewed, and the medication was not returned or disposed of as required.
Plan of Correction: Per the Center: "Medication was returned to the parent."
