Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment.

Staff #2, who had been employed for two months, did not have a TB screening completed within 30 days of their employment start date.

When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet.

At the time of the inspection, the medication authorization forms for two of Child #3?s onsite medications had expired.
Plan of Correction: Parent has been notified that medication authorization for child #3 has expired. Parent will provide updated authorization and new medications.
