During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced monitoring inspection was initiated on 2/25/2026 and completed on 4/2/2026 with a local agency in response to a self-report received by the licensing office on 2/20/2026, relating to an injury. The inspectors reviewed 1 child?s record and 5 staff records on-site. This inspection included document review and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Though not a part of the original self-report, violations that were not reported were found related to administration and are documented on the violation notice issued to the program.

Violations have been cited related to noncompliance, resulting in an injury to a child while under the supervision of the facility that required outside medical treatment. Please see the violation notice for more details.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/8/2026. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.

At the time of the inspection, Staff #2's tuberculosis screening was overdue for 14 days.
Plan of Correction: A new Updated TB has been Submitted.
Standard 8VAC20-780-400-B
Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.

On Tuesday, February 10, 2026, during snack, Staff #1 asked Child #1 (age 2) to use the restroom. Staff #1 stated the child refused, stating ?no.? Staff #1 reported that they used both hands to pull the child by the left elbow and their left hand to take the child to the bathroom, which is neither constructive in nature nor age and stage appropriate. While being pulled toward the bathroom, the child resisted by pulling back toward the snack table. After snack, Child #1 complained of pain in the left arm.
Plan of Correction: Effective immediately, staff will continue to use constructive, age-appropriate behavioral guidance focused on redirection and support during transitions.
Staff #1 has received additional guidance and support on best practices for assisting children, including using verbal cues and encouragement during routines such as toileting.
All staff will participate in a refresher on positive behavior guidance, and management will continue to provide support and monitor classrooms to ensure consistent implementation.
Standard 8VAC20-780-410-1
The following actions are forbidden: roughly handling a child.

Plan of Correction: Effective immediately, staff will use supportive, age-appropriate strategies to guide children through daily routines, ensuring all interactions remain gentle and respectful.
Staff #1 has been provided with additional guidance on appropriate transition techniques, including allowing extra time, offering choices, and using verbal prompts during toileting routines.
A center-wide refresher will be conducted on positive behavior support and appropriate physical interactions with children. Leadership will continue to monitor classrooms and provide ongoing coaching to ensure consistency in practice.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries, including all required information, in which entries are made the day of occurrence.

Child #1's report did not include the date and time the parents were notified or the method by which the parents were notified. Additionally, the date of injury was documented incorrectly.
Plan of Correction: Effective immediately, the center will ensure that all injury reports are completed accurately on the day of occurrence and include all required information, including:
? Date and time of the injury
? Date, time, and method of parent notification
? Details of the injury and any treatment provided
Staff have been retrained on proper documentation procedures for both minor and serious injuries. Supervisors will review each report for accuracy before filing to ensure compliance.
Ongoing monitoring will be conducted to ensure all future injury reports are complete, accurate, and submitted in a timely manner.
Standard 8VAC20-780-80-B
Centers shall inform the department's representative as soon as practicable, but not to exceed two business days, of any injury to a child that occurs while the child is under the supervision of the center and requires outside medical attention.

Child #1 received outside medical treatment for an injury sustained while in care at the center
on Tuesday, February 10, 2026. The parent notified the center on Thursday, February 12, 2026, but the center did not notify the Department until eight days after being notified by the parent.
Plan of Correction: Effective immediately, the center will ensure that any injury requiring outside medical attention is reported to the Department within the required two business days.
Administration has reviewed reporting procedures and timelines with all leadership staff to ensure a clear understanding of reporting requirements. A standardized internal reporting process will be implemented to track incidents and ensure timely notification.
Moving forward, all incidents involving outside medical care will be reported promptly upon notification, and documentation will be maintained for compliance. Leadership will monitor reporting practices to ensure adherence to regulations.
