Inspector Notes

An unannounced on-site monitoring inspection was conducted on 7/12/2022. There were 12 children, ages 4 months-3 years, and 5 staff members present during the inspection. The inspector observed the following: free choice activities outside, large motor play outside, small group and large group and lunch. A total of 5 children?s records and 4 staff records, were reviewed. The inspector arrived at 10:30 and departed the program at 1:20.

Based on review of nine authorizations of sunscreen, the center failed to ensure that all required components were obtained.

Evidence:
1. Eight of the nine authorizations for sunscreen did not documented the adverse reactions.
Plan of Correction: The director will have the parents to complete the missing information ASAP.
