Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 09/09/2024 as a part of a self-reported incident that occurred on 7/1/2024. The on-site inspection began at 10:20 AM and ended at 12 PM. The inspector reviewed compliance in the areas listed above. There were 16 children present and 6 staff. The inspector interviewed 3 staff onsite. This inspection included: document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on review of written statements and interviews with staff, the center failed to ensure that under 10 years of age shall always be within actual sight and sound supervision of staff.
Evidence:
Verified through written statements and interviews with staff, child # 1 was out of sight and sound supervision for approximately 2 to 3 minutes. The children were lined up to enter the building from the outdoor play area. Child # 1 wondered from the entrance to the playground to the front of the building. Once realized child # 1 was missing, staff # 2 went out the front door and staff # 1 went around the back. The child was located in the front of the building around the shrubs.

Plan of Correction: A temporary gate has been placed out side of the playground fence. This will prevent children from wondering away from line. A permanent gate will be added as soon as the center can.
