Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 09/09/2024. The on-site inspection began at 10:20 AM and ended at 12 PM. The inspector reviewed compliance in the areas listed above. There were 16 children present and 6 staff. The inspector reviewed 5 children?s records and 2 staff records onsite. This inspection included: document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Based on record review, the center failed to ensure that the separate record for each
enrolled child shall contain all the elements as required by the standards.

Evidence:

The record for Child #4 did not include the following information: address for one of the
two designated people to call in an emergency if a parent cannot be reached.
Plan of Correction: The director will obtain the missing information from the parent and will monitor.
