Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A non mandated monitoring inspection was initiated on 6/8/21 and concluded on 6/10/21. The provider was contacted by telephone to initiate the inspection. The purpose of this inspection was to assess the care of children and daycare activities during the conditional period. There were 5 children (13 points) and 2 caregivers present. During the virtual portion of the inspection, the children were observed playing indoors and eating lunch. The lunch menu included a mixed vegetable stew with rice and milk. A discussion was held about maintaining the approved capacity and points for the family day home when accepting part time children. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 1 caregiver's record submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. 2. The provider stated that she did not obtain any paperwork for Child # 1. No documents were available to review for Child # 1 during the inspection.

1. Caregiver # 1's first day of employment was 5/24/21.
2. The provider stated on 6/10/21 that Caregiver # 1 had not completed the SDS prior to employment, and will have it completed this day.

1. Caregiver # 1's record indicated that her first day of employment was 5/24/21.
2. Caregiver # 1's fingerprint results were obtained on 6/7/21.
