Inspector Notes
An unannounced monitoring inspection was conducted from 10:40 am to 12:15pm. There were 9 children (24 points) with 3 caregivers. The children were observed playing with toys in small groups. Handwashing was observed before lunch. There were no medications on site. A walk through of the family day home was conducted. 3 children's records and 3 caregiver records were reviewed.

Violations
3Evidence: The provider's first aid training expired on 1/25/22.
Evidence: One infant was observed using an infant walker.
Evidence:
1. There were two infants in care during the inspection.
2. One infant bottle was found sitting on a shelf without the child's name and date.
