Inspector Notes

An unannounced monitoring/renewal inspection was conducted today between 11:10am and 12:45pm. There was 1 children present with the Provider for a total of 2 points within the supervision guidelines. The child was observed playing freely and socializing with a home visitor. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, fire drill log, menu and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice.

Based on record review, the family day home did not obtain documentation of additional immunizations every six months for children under the age of 2 years.
Evidence: Child #3 (21 months) most recent documented immunizations are dated 11/20/2022.
Plan of Correction: I'll see the family tomorrow and I'll let them know that I need an updated immunization record.
