Inspector Notes

An unannounced monitoring inspection was conducted 4/10/2023 between the hours of 9:45am through 10:40am. There were no children present with the provider at the time of inspection. One staff files and one household member?s files were reviewed. A complete inspection of the programming space, staff records, emergency drills, menu, attendance, and emergency procedures were completed today. The provider advised there was no medication on site today. There was an abundant supply of age appropriate materials and toys. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the provider did not ensure that all caregivers and adult household members submit to an updated tuberculosis (TB) screening or test at least every two years from the date of the first screening.

Evidence: Staff #1's most recent TB test was completed 2/18/2021. Adult Household Member #1's most recent TB test was completed 3/3/2021.
Plan of Correction: Scheduled appointment with doctor to update TB test.

Based on record review and provider interview, the provider did not maintain documentation of monthly emergency evacuation drills.

Evidence: There was no documentation of emergency evacuation drills conducted in January, February, or March of 2023.
Plan of Correction: Posted all emergency logs in a visible location. Initiated and completed an emergency evacuation drill.
