Caregivers are required to obtain a minimum of sixteen clock hours of training annually in
areas relevant to their job responsibilities, in addition to the first aid and CPR training.

1. Caregiver #1 only had documentation for two out of sixteen clock hours of training annually.
2. Caregiver #2 did not have documentation on file to show that they obtained a minimum of sixteen clock hours of training annually.
Plan of Correction: Will complete all required courses in a timely manner.

Soiled disposable diapers and wipes must be disposed of in a leak-proof or plastic lined storage system that is either foot operated or used in such a way that neither the caregiver's hand nor the soiled diaper or wipe touches the exterior surface of the storage system during disposal.

The provider did not have a foot operated storage system for disposal of soiled diapers and wipes.

Medication, except for those prescriptions designated otherwise by a written physicians order, including refrigerated medication and medications for caregivers and household members, shall be kept in a locked place using a safe locking method that prevents access by children.

There were two bottles of prescribed medication that were observed up high in an organizer that was not locked on the second floor of the home. Children were observed to be able to have access to the second floor.
