Inspector Notes
An unannounced monitoring inspection was conducted 2/7/2023 from 11:13am to 1pm. There were 52 (7mos to 5yrs) directly supervised by 10 staff. The physical plant, 5 staff records, 5 children?s records, 2 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, listening to stories, playing outside, eating lunch, and preparing for and taking naps. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Violations
4Evidence: observed a hole in the wall in the Pre-K 2 classroom, large enough for children's fingers to enter.
Evidence: observed in the Pre-K 1 classroom, soap water and Lysol spray kept in a locked cabinet with the key kept in the lock. Observed in the Three's classroom hazardous substances in a locked cabinet, with a key in the lock.
Evidence: observed that the first aid kit was missing 1 triangular bandage.
Evidence: reviewed 5 staff records and found that Staff 2 did not have documentation of an emergency contact.
