Inspector Notes
An unannounced monitoring inspection was conducted on 9/14/2023 at 2:05pm. There were 41 (infants to pre-K) directly supervised by 6 staff. The physical plant, 1 staff record, 5 children?s records, no new children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed napping, waking up from nap, and interacting and doing activities with staff. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Violations
3Evidence: observed the following:
1) a hole in the floor of the Twos classroom
2) a storage box on the playground that was broken
3) a hole in the fence of the 2s playground
4) peeling paint along the outside windows facing the playground
Evidence: observed that the preschool play equipment had approximately 2-3 inches of resiliency covering the fall zones, and not the recommended minimum six inches.
Evidence: reviewed 5 children's records and found that Child 5 did not have an address for one of their emergency contacts.
