Repeat violation:
Where playground equipment is provided, resilient surfacing shall comply with minimum safety standards when tested in accordance with the procedures described in the American Society for Testing and Materials standard F1292-99 as shown in Figures 2 (Compressed Loose Fill Synthetic Materials Depth Chart) , and shall be under equipment with moving parts or climbing apparatus to create a fall zone free of hazardous obstacles.

1. On the infant playground there was a climber with a slide that was within 4 feet of the fence.
2. On the two year old playground a climber with a slide was within 4 feet of a picnic table and within 5 feet of a second table.
3. On the big playground was a climbing caterpillar that was within 4 feet of the sidewalk.

All these climbers required a 6 foot fall zone that is clear of all barriers/objects other than the resilient surface.
Plan of Correction: We will talk to the teachers about the need to maintain adequate fall zones at all times and that moving the play equipment interferes with maintaining at least six feet of fall zone at all times.

Prepared infant formula shall be refrigerated, dated and labeled with the child's name.

Two infant bottles in the refrigerator were not dated.
Plan of Correction: We will date the bottles and remind staff to ensure parents are dating the bottles and if not then it will be the teacher's responsibility to do so.
