Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 07/31/2024 and completed on 07/31/2024. The on-site inspection began at 9:30 am and ended at 11 am. The inspector reviewed compliance in the areas listed above. There were 133 children present and 29 staff. The inspector reviewed 10 children?s records (today) and followed up on 3 staff records that were reviewed on-site on 07/03/2024. This inspection included:

? document review of injury logs, policies and procedures, emergency drill logs, medication administration logs, authorization forms,
? tour of the facility (both inside and outdoor equipment), and
? interviews.

Information gathered during the inspection determined non-compliance with an applicable standard, code, and the violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-B-4
Based on review, the Child Day Center (CDC) did not obtain a copy of the results of a criminal history record information check (CRC) from any state in which the individual has resided in the preceding five years prior to their start at the CDC.

Evidence -
On the date of inspection (07/31/2024), the results of a CRC was not available for the following staff from the following states:
1. Staff #2 (start date - 06/24/2024) from the State of Florida
2. Staff #3 (start date - 06/24/2024) from the State of Illinois
3. This same code was previously cited during an inspection conducted on 07/03/2024.
Plan of Correction: These CRCs will be requested. Once they are obtained, findings will be sent to the Licensing Inspector for review.
Standard 8VAC20-780-140-A
Based on review, 1 of 10 children files indicated that each child did not have documentation of a physical examination on file within 30 days of their start date within the CDC.

Evidence -
1. On the date of inspection (07/31/2024), documentation of a physical examination for Child #6 was not available for review.
2. The start date for Child #6 was listed as 06/24/2024.
Plan of Correction: The Parent of Child #6 will be contacted in order to obtain this documentation.
