Inspector Notes

An unannounced monitoring inspection was conducted today, April 10, 2023. The Inspector was on site from 10:30 am until 1:15 pm. The focus of the inspection was to determine whether the provider had corrected or is in the process of correcting previously cited violations in the areas of standards referenced above. This inspection yielded no violations of applicable laws or regulations.

Pursuant to 8VAC20-820-410, General Procedures and Information for Licensure, the provider requested that the Department consider entering into a Consent Agreement in lieu of adverse action to Deny Renewal Application For A License. On March 31, 2023, the provider submitted a final Compliance Plan to address the standards and law documented on a Notice of Intent to Deny Renewal Application For A License issued, November 28, 2022. The provider demonstrated that they have followed their Compliance Plan actions.
