Inspector Notes
The licensing inspector conducted an unannounced complaint inspection in response to a complaint received by the department on April 5, 2022 regarding staff to children ratios. The investigation was conducted on 04/21/2022. On 04/21/2022, the inspector was on site from 10:00 am to approximately 12:45 pm. Interviews were completed, observations were made at the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports the allegations, therefore the complaint is determined to be valid. Any violations not related to the complaint, but identified during the course of the investigation can be found on the violation notice.

On April 21, 2022, there were ten children, ranging from four months to 17 months in age, being supervised by two staff exceeding the requirement for one staff to every four infants.
Evidence:
On April 21, 2022, there were nine children, ranging from 16 to 24 months in age, being supervised by one staff member exceeding the requirement for one staff to every five children.
Evidence:
On April 21, 2022, there were ten children, two years in age, being supervised by one staff member exceeding the requirement for one staff to every eight children.
