3Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance. Child #8, 15 months in age, was observed holding a bead from a hair tie that had broken. A second bead was observed on the floor where multiple children under the age of 16 months were playing posing a choking hazard.
Plan of Correction: Per the director: Staff will observe children with beads to ensure they are secure, and frequently throughout the day.
Standard 8VAC20-780-370-5
There shall be a flexible daily schedule for infants based on their individual needs. Infants, who cannot turn themselves over and are awake, shall be placed on their stomachs a total of 30 minutes each day to facilitate upper body strength and to address misshapen head concerns. Child #4, three months in age and not able to turn over, did not have documentation of tummy time for the previous week.
Plan of Correction: Per the director: Tummy time has been done, it just wasn't documented. Going forward we will document daily.
Standard 8VAC20-780-40-M
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan. This list shall be dated and kept confidential in each room or area where children are present. The allergies, sensitivities, and dietary restriction list was not dated. Child #6 and child #7, both with allergy care plans, were not included on the list.
Plan of Correction: The director updated the allergy list with the missing children and dated it.
