Inspector Notes
An unannounced monitoring inspection was conducted on-site March 30, 2023. The director was available during the inspection. There were 17 children present, ranging in ages from 7 weeks to 4 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.
Violations
4Evidence: 1. The record of staff #1, hired 02/27/23, did not contain documentation of a sworn statement. 2. Staff #4 confirmed the sworn statement was not completed.
Evidence: 1. An unlocked double-door hallway closet contained several cleaning materials, paint, and insecticides. There were materials both on the floor of the closet within reach of children and on shelves. There were approximately 25 various cleaning solutions and approximately 12 paint cans. There was also lighter fluid, shaving cream, weed killer, and carpet cleaner observed. The labels stated "keep out of reach of children" and "caution" or "warning."
2. The hallway bathroom, used by children, contained Zep Refreshen spray in an unlocked cabinet. The label on the spray stated "keep out of reach of children" and "warning."
3. The bathroom used by two year old's contained bleach water on the diaper table. No diapering was occurring. The label on bleach states "keep out of reach of children" and "caution."
Evidence: 1. The kitchen, which is adjacent to a classroom with no prevented access by
children, contained a drawer with several butcher and serrated knives. There were also
two knives in the strainer by the sink. 2. The two year old classroom contained a
pairing knife in the sink.
Evidence: 1. The record of child #1 (enrolled 8/16/18), did not contain documentation of an annual review. The last record review was documented as 06/01/21.
2. The record of child #3 (enrolled 8/24/21) did not contain documentation of an annual review. The last record review was documented as 08/24/21.
