Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and interviews.

A renewal inspection was initiated on 11/9/21 and concluded on 11/12/21. The director/provider was contacted by telephone and a virtual inspection was conducted. There were 18 children present, ranging in ages from 5 to 10, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 2 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the center failed to maintain a written record of children's serious and minor injuries in which the record contains a staff and parent signatures or two staff signatures.

Evidence:

Three injury reports were reviewed. The three injury reports reviewed only contained one signature.
Plan of Correction: Staff will be retrained on completing an injury report to ensure a staff and parent signatures or two staff signatures are included on each injury report in the future.
