The medication authorization shall be available to staff during the entire time it is effective. One child's over-the-counter medication did not have parent or physician authorization.
Plan of Correction: A medication permission form will be obtained. In the future the director will ensure a permission form is available for all medication as they are brought to the center.
Standard 8VAC20-780-510-G
Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. One child's over-the-counter medication was not labeled with the dosage.
Plan of Correction: The medication will be documented with the dose. In the future the director will ensure medication is labeled upon being brought to the center.
Standard 8VAC20-780-60-A-8
A written care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. One child is listed as having a food allergy. The center does not have an allergy care plan for the child.
Plan of Correction: An allergy action plan was obtained dated 12.10.25. In the future the director will ensure any child that has a listed allergy has an allergy action plan on cite.
