Inspector Notes

A mandated monitoring inspection was conducted on May 18, 2022 from 3:15 P.M. to 4:45 P.M. There were 26 children present, ranging in ages from six years old to twelve, with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of children's records, the center failed to obtain immunization record prior to the first day of enrollment.

Evidence:
1. Child #3's file did not contain documentation of immunizations. Date of enrollment for Child #3 was 08/25/2021.
2. Staff verified the record did not contain the documentation of immunizations.
Plan of Correction: Administration will request the immunization record from the parent. In the future, children's records will be reviewed by two staff members to ensure all documentation is on file.
