Inspector Notes

An unannounced, on-site renewal inspection was initiated on and completed on 5/28/2025. The on-site inspection began at 8:45 am and ended at 10:00 am. The inspector reviewed compliance in the areas listed above. There were 12 children present for a point total of 27 and four caregivers. The inspector reviewed five children?s records and three caregiver records on-site. This inspection included document review, tour of the facility, interviews, observations and measurements.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
A tuberculosis (TB) screening is required every two years from the last screening.

An updated TB screening for Caregiver #2 and #3 was not completed and was overdue by four months.
Plan of Correction: Provider will ensure caregiver's get TB tests completed every two years from the last test.
Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required in each child's record to ensure the information is correct and shall obtain the parents signed acknowledgement of the review.

Records for children #1, #2 and #3 did not have a parent's signature for an annual review of the emergency contact information. Child #1 and #2 have been in care for over a year. Child #3 has been in care for three years.
Plan of Correction: Provider will update missing reviews and ensure yearly reviews of children's record are completed and documented.
