22.1 Background Checks

Inspector Notes

An unannounced inspection was initiated on 9/26/2025 and completed on 10/9/2025 with the Virginia Department of Behavioral Health in response to a complaint received by the licensing office on 9/23/2025 relating to allegations of Administration. The inspector reviewed two household member's file electronically on 10/2/2025. This inspection included document review, tour of the facility, interviews and observations.

The preponderance of evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found and are documented on the violation notice issued to the program.
