Inspector Notes

A monitoring inspection was initiated and concluded on 4/11/2024. There were no children present during the inspection. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies. A total of 6 staff records and 5 children records were reviewed. The inspection started at 12:55pm and concluded at 2:05 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the center did not ensure all required out of state background check were completed for staff who lived out of the state of Virginia within the past 5 years. .

Evidence:
Staff #2 lived out of the state in the past five years. Staff #2 lived in Texas, Louisianna and Mississippi. The center did not to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from Louisianna , Texas and Mississippi. Staff #2 hire date 08/29/2023.

Staff #3 lived out of the state in the past five years. Staff #3 lived in Texas and California. The center did not to obtain a copy of the results of a criminal history record information check, a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from Texas and California. Staff #3 hire date 08/29/2023.

Staff 2 TX criminal and Staff 3 TX check and sex offender he submitted via email 4/15/2024. Child Abuse and Neglect mailed week of 4/22/2024.

Staff #2 LA sexoffender and child abuse and negelct submitted via email4/15/2024. Criminal check being mailed week of 4/22/2024.

Staff #3- CA sex offender check submitted via email 4/15/2024. CA Crimnial and Child Abuse and Neglect will be mailed the week of 4/22/2024.

Staff #4- Illinois sexoffender submitted via email 4/15/2024. IL criminal / child abuse and neglect mailed week of 4/22/24.

Based on record review and staff interview, the center did not ensure central registry results for staff were recieved by the 30th day of employment.

Evidence: Staff #1 employment date 8/29/2023. The center did not have documentation of Staff #1 central registry results during the inspection. Staff#1 was employed for more than 30days.
Plan of Correction: DSS was contacted by phone and email multiple times. Bank said check was cashed and DSS has not provided background check being mailed 4-22-2024.
