Inspector Notes

An unannounced inspection was initiated on October 9, 2024 and completed on October 30, 2024 in response to a complaint received by the licensing office on September 19, 2024 relating to allegations of program operations. The inspector reviewed 3 children records and 2 staff records onsite on October 30, 2024. This inspection included document review, tour of the facility and interviews. The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to general record keeping and are documented on the violation notice issued to the program.

Records and reports on children and staff required by this chapter shall be maintained and made accessible for two years after termination of services or separation from employment unless specified otherwise.
The record for child #3, who disenrolled from the program in August of 2024 was not available for review.
Plan of Correction: Purchase a storage bin and store the old files in the bin and keep for the amount of years that will be required.
