Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, document review and a virtual tour of the program.

A renewal inspection was initiated on 11/15/2021 and concluded on 11/17/2021. The provider was contacted by telephone and a virtual inspection was conducted. There were 21 children, ages 12 months-5 years, and 6 staff members present during the inspection. The inspector reviewed compliance in the areas of administration, staff qualifications and trainings, staffing and supervision, programs, physical plant, emergencies and nutrition. A total of 3 children?s records, 3 staff records and 1 manager records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review and discussion with staff, the center failed to obtain a copy of the results of a sex offender registry check from any state in which the individual has resided in the preceding five years.

Evidence: The record for Staff 3 contained documentation that Staff 3 had resided outside of the state of Virginia within the preceding five years. The record for Staff 3 contained a documented hire date of 11/04/2021. There was not a sex offender registry check in the record for Staff 3 during the 11/17/2021 inspection.
Plan of Correction: The sex offender registry check was obtained during the 11/17/2021 inspection and was placed in the staff record.
