Inspector Notes
An unannounced, on-site monitoring inspection was initiated on 10/30/2024 and completed on 11/05/2024. The on-site inspection began at 11:35 PM and ended at 1:20 PM. The inspector reviewed compliance in the areas listed above. There were 31 children present and 8 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included document review, tour of the facility and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1The record for Child # 4 has been enrolled for 3 months and did not have the complete addresses for the two designed people to call in an emergency if the parent cannot be reached.
