Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A 2 month monitoring visit was conducted during the Conditional License. The director was contacted by telephone and a virtual inspection was conducted. There were 30 children present, ranging in ages from infant to 4 years old, with 10 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant (including the new playground equipment), staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 2 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and observation, the center did not follow the medication procedures for obtaining the physician signature for a long term authorization.
Evidence:
Child #1's authorization did not contain the physician signature as the doctor only signed the allergy action plan.
Plan of Correction: The State model form will be given to the parent to have the doctor complete the long term authorization.
