Inspector Notes

An unannounced monitoring visit was conducted. All rooms were in ratio with direct care staff. Observed infant care, diapering, transition to the outside play area, activities, reading books and free play. A sample of staff, children and medication records were audited. Emergency preparation for drills was current. Staff trainings including first aid and CPR, Medication Administration Training (MAT certification) as well as Daily Health were current for multiple staff. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the center did not obtain updated immunization record for a child under two years of age.
Evidence:
Child #1's last immunization record was dated 8/2021 and the child is under two year of age.
Plan of Correction: The center will request the parent obtain an updated immunization record.
Standard 8VAC20-780-280-B
Based on observation, a hazardous substance was not kept in a locked place using a safe locking method to prevent access by children.
Evidence:
A spray can of Fabreze air freshener was hanging on a hook above the diapering table in classroom 9.
Plan of Correction: All hazardous materials will be kept locked up in each classroom.

Based on observation, a non-absorbent surface for diapering is not in use.
Evidence:
The changing pad in classroom Infant A has an approximate 1 inch tear.
Plan of Correction: A new changing pad will be placed in the classroom for diapering.

Evidence:
1. Child #1 is missing a second emergency contact address.
