Inspector Notes
An unannounced monitoring visit was conducted. All rooms were in ratio with direct care staff. Observed infant care, diapering, transition to the outside play area, activities, reading books and free play. A sample of staff, children and medication records were audited. Emergency preparation for drills was current. Staff trainings including first aid and CPR, Medication Administration Training (MAT certification) as well as Daily Health were current for multiple staff. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Child #1's last immunization record was dated 8/2021 and the child is under two year of age.
Evidence:
A spray can of Fabreze air freshener was hanging on a hook above the diapering table in classroom 9.
Evidence:
The changing pad in classroom Infant A has an approximate 1 inch tear.
Evidence:
1. Child #1 is missing a second emergency contact address.
2. Child #2 is missing an allergy action plan for a diagnosed food allergy
