Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on September 21, 2021 and concluded on September 22, 2021. The director was contacted by telephone and a virtual inspection was conducted. There were 28 children present, ranging in ages from 6 weeks to 4 years, with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records and 7 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. You will need to specify how the deficient practice will be or has been corrected. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Please do not use staff names; list staff by positions only.
Violations
1Evidence: 1. The record of Staff #1 (DOH 4/20/21) contained telephone references dated 5/3/21.
2. In an interview with Administration regarding positions in the classroom, it was reported that Staff #1 is a program lead. The record of Staff #1 did not contain documentation that is required to meet program leader qualifications.
2. Conversation with director advising all staff training documentation is to be filed in the staff folder as soon as training is completed to prevent loss. In addition, an electronic record is to be created for backup purposes. Steps taken to correct issue: 1. Training document updated to note record hours and copy of training certificate are to be filed immediately. 2 Electronic record of training kept for future reference. Measure to prevent from happening again: director will send copy of electronic record to home office for reference. Person responsible for monitoring: owner or office clerk.
