Inspector Notes

An unannounced monitoring inspection was conducted on 2/2/2023 with center director. There were 59 children present, ranging in ages from 6 months to 5 years, with 11 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. There is no medication being administered at this time. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review, the center failed to provide a tuberculous (TB) test with documentation that includes the date the test was given and results of the test.
Evidence:
1. Staff #5?s tuberculous (TB) test did not provide documentation of the date when the test was given and the results of the test. Staff #5?s date of hire 10/31/2022.
Plan of Correction: Will get the doctor to sign 2/2/2023.

Based on observation, the center failed to ensure that all prepared infant formula was labeled with the date.
Evidence:
1. 4 prepared bottles of formula, were not labeled with the date.
Plan of Correction: Date will be placed on the bottle.

Based on observation and interview it was determined the center did not ensure that the findings of the most recent inspection of the center were posted on the premises.
Evidence:
1.The results for the most recent inspection were not posted in the center. Posted inspection was dated 1/20/2022
2. Staff confirmed that the most recent inspection results were not posted.
