Inspector Notes

An unannounced monitoring inspection was conducted on August 16, 2023. The inspection began at approximately 1:05 PM and concluded at 3:00 PM. The center director was available for the inspection. There were 79 children present, with 13 staff supervising. The children were observed napping and playing with toys. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Based on observation and interviews, it was determined that there was an insufficient amount of resilient surfacing in the use zone of the playground equipment and the area surrounding equipment did not encompass sufficient area to include the child's trajectory in the event of a fall while the equipment is in use.
Evidence:
1. There is only 2 inches of mulch in the fall zones of the play structure on the school age playground, where 6 inches is required.
2. Staff confirmed that there is less than 6 inches of resilient surfacing on the playground.

Based on document review, the center failed to ensure that the written record of children?s serious and minor injuries in which entries are made the day of occurrence contained all of the elements as required by the standards.
