Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on August 20, 2024. The on-site inspection began at 10:20 AM and ended at 12:50 PM. The inspector reviewed compliance in the areas listed above. There were 60 children present and 14 staff. The inspector reviewed 6 children?s records and 6 staff records on-site. This inspection included a tour of the facility, interviews, observations and document review.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-240-C
Based on a review of six (6) staff records and interview, the center did not ensure that staff completed the required recognizing child abuse and neglect training no later than 7 days of the date of assuming job responsibilities.

Evidence:
1. The record for staff #1 (hire date 2/10/2024), staff #2 (hire date 5/13/2024), and staff #3 (hire date 5/16/2024) did not contain documentation of completed recognizing child abuse and neglect training.

2. The director verified that the training for recognizing child abuse and neglect had not been completed for staff #1, staff #2, or staff #3.
Plan of Correction: Staff will be updating training by Wednesday 8/21/2024.
Standard 8VAC20-780-500-A
Based on observation, the center failed to ensure that children's hands shall be washed with soap and running water after toileting and any contact with blood, feces, or urine.

Evidence:
In the infant room, Staff # 6 was observed changing a diaper. Staff # 6 did not ensure that the child?s hands were washed with warm running water and soap.
Plan of Correction: Retrain staff on changing procedures.
