An unannounced, on-site modification inspection was initiated and completed on April 25, 2025. The on-site inspection began at 10:05 am and ended at 10:45. The inspector reviewed compliance in the areas listed above. There were no children or staff present in the new addition. This inspection included document review, a tour of the facility, interviews, observations, and measurements.
