Inspector Notes
An unannounced monitoring inspection was conducted from 1:00 pm - 2:00 pm. There were 8 children in care with the provider and one assistant for a total of 15 points. The children were observed eating lunch and preparing for rest. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and two staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on review of one assistant record it was determined that not all records were complete. Evidence: The record for Staff A (start date: 06/3/21) did not contain documentation of name, address, verification of age, job title, date of employment, name and address of an emergency contact, and documentation of education,
Plan of Correction: The missing documentation will be completed for the file.
Standard 8VAC20-800-180-A
Based of review of one assistant record it was determined that not all records contained documentation of a TB (tuberculosis) screening every two years. Evidence: The most recent TB screening on file for staff A is dated 04/08/2021.
Plan of Correction: Staff A will obtain a new TB screening for the file.
Standard 8VAC20-800-200-A
Based on review of one assistant record it was determined that the record did not contain documentation of orientation by the end of their first week of employment. Evidence: The record for Staff A (start date: 06/03/21) did not have documentation of orientation training.
Plan of Correction: The provider will complete orientation training with staff A.
Standard 8VAC20-800-250-A
Based on observation it was determined that not all hanging appliance cords were kept out of reach of children. Evidence: Child A (Age 2) was observed in a crib that was directly under a television mounted on the wall. There were three cords under the television that were accessible.
Plan of Correction: The crib was moved during the inspection.
