An unannounced inspection was initiated and completed on March 13, 2025 in response to a complaint received by the licensing office on March 13, 2025 relating to allegations of staffing/supervision and special care provisions. This inspection included document review, tour of the facility, interviews, and observations.

The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
