Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on April 28, 2025. The on-site inspection began at 9am and ended at 10:15am. The inspector reviewed compliance in the areas listed above. There were 2 children present and 1 staff. The inspector reviewed 2 children?s records, 2 household member?s records, and 2 staff records on-site. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.057-A
The program is required to submit to VDH and implement a plan to test potable water for lead. The program did not have evidence of a testing plan submitted to VDH and/or the program did not have evidence of the implementation of the required testing plan.

Standard 8VAC20-800-30-A
The provider shall ensure compliance with these standards and the terms of the
current license issued by the department and with relevant federal, state or local
laws, and other relevant regulations. Child #1 was enrolled at 9 months old when the age range for children in care are licensed for ages 1 years old to 12 years old.
