During the inspection, the inspector reviewed the areas listed above, to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on August 27, 2025. The on-site inspection began at 10:10am and ended at 11:10am. The inspector reviewed compliance in the areas listed above. There were 5 children present and 1 staff. The inspector reviewed 5 children?s records, 2 household member?s records, and 1 staff record. This inspection included document review, tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-770-60-B
A child's record shall contain emergency contact information including name, home address, and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached. Three of the five children?s records reviewed did not have one of the two emergency contacts listed.
Plan of Correction: The provider responded with the following: The emergency contacts will be obtained by the parents and placed in the record.
Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination within 30 days after the first day of attendance. Two children did not have their physical in their record and have been enrolled at the FDH for over 2 months.
