Inspector Notes

An unannounced, on-sit e renewal inspection was initiated and completed on October 23, 2024. The on-site inspection began at 9:00am and ended at 10:00am. The inspector reviewed compliance in the areas listed above.There were 5 children present and 2 caregivers; the point total was 18. The inspector reviewed 5 children?s records and 3 caregiver/household member(s) records on-site. This inspection included document review, tour of the facility and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Violations

Standard 8VAC20-800-60-B
A child's record shall contain all the required information.
The record for child #1 did not contain the address for one emergency contact and the name, address and phone number for a second emergency contact for five months.
Plan of Correction: I will get the emergency contact information for the child. I will ensure I always get the information for two contacts.
Standard 8VAC20-800-680-B
Caregivers shall ensure that children's hands are washed with liquid soap and warm running water after diapering.

After diapering, child #1 did not have their hands washed.
Plan of Correction: I will make sure to wash the infants hands after each diaper change.
Standard 8VAC20-800-690-D
The diapering surface shall be non absorbent and cleaned and sanitized after each use.

The diaper surface has a fabric covering that is absorbent and can not be cleaned and sanitized after each use.
Plan of Correction: I thought since I use a covering I didn't need to clean and sanitize the diaper surface. I will remove the covering and clean and sanitize after each diaper change.
Standard 8VAC20-800-780-B
Annually, the provider shall review with the parent the emergency contact information required in to ensure the information is correct, and obtain the parent's signed acknowledgment of the review.
The record for child #2 has not had an annual parental review for over five months.
Plan of Correction: I will have the parent review the child's record and document they reviewed it.
