Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 4/8/2026. The on-site inspection began at 11:10 am and ended at 12:05 pm. The inspector reviewed compliance in the areas listed above. There were 10 children, including provider's own child present and 2 caregivers; the point total was 32. The inspector reviewed 5 children?s records and 4 caregiver/household members records.
This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-550
A playpen, Pack N Play shall not be used for designated sleeping area.
A Pack N Play is used for child # 2, age 3 1/2 months for sleeping. This has been the infants sleeping area for the past 2 months.
Plan of Correction: A crib will be pulled from storage to be used for the infant.
Standard 8VAC20-800-780-B
Annually the provider shall review with the parents the emergency contact information to ensure that information is correct.
The information for child # 1 did not have annual review of emergency information, The review was over 18 months past due.
Plan of Correction: Parents will review the emergency information.
Standard 8VAC20-821-170-B
Household members 14 years of age and older must obtain the results of a central registry search within seven days of reaching 14 years of age.
Household Member #1 did not have central registry results during the inspection and was over due by 6 months.
Plan of Correction: Central registry results will be requested.
