Inspector Notes

This inspection was conducted by staff using an alternative remote protocol, including phone interview and record review. A virtual observation or on-site inspection was not conducted at this time.

An unannounced monitoring inspection was conducted at 10:35am. The provider was contacted by phone to initiate the inspection, and given a list of items required to complete the inspection. There was 5 children present = 14 points with the provider. Information gathered during the inspection determined there were areas of non-compliance with applicable standards or laws reviewed. Based on records reviewed, it was determined that 2 of 2 children's records did not include required information.

Evidence:

1. Child #6 - There was no documentation of medical insurance and emergency medical authorization signed by the parent. The emergency medical authorization form on-file was blank and not signed by the parent.

2. Child #7 - There was no documentation of medical insurance, emergency medical authorization signed by the parent, and notice of liability insurance form.
Plan of Correction: The Provider will have parents complete the required forms, and in the future review paperwork to ensure completion prior to enrollment.
