Inspector Notes

An unannounced monitoring inspection was conducted on 5/7/2024 from 10:00am to 11:11am. There were 12 child care, 1 resident child (28 points) directly supervised by 4 caregiver(s). The home, 1 caregiver's record, 3 children's records, household member's records, evacuation drills, emergency supplies, and policies were inspected. There were no children's medications on site today. Children were observed playing freely outside and inside the play room. There was an abundant supply of books, toys, and materials for the children. The home was clean and organized. Areas of non-compliance are identified in this report. Based on review, the provider did not ensure that each report shall include the date of the physical examination and dates immunizations were received and shall be signed by a licensed physician, the physician's designee, or an official of a local health department.
Evidence: reviewed 3 children's records and found that Child 1 did not have documentation of a signed physical examination, and Child 1 and 3 did not have documentation of a signed immunization record.
