During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on May1. A POC submitted after this date will not appear on the public website.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on April 24, 2025 and completed on April 24, 2025, as a part of the current licensure period. The on-site inspection began at 9:10am and ended at 10:00am. The inspector reviewed compliance in the areas listed above. There were 9 children present and 4 caregivers; the point total was 21. The inspector reviewed 1 child file and 4 caregiver/household members files on-site.
This inspection included: document review, tour of the facility, interviews, and observations.

Violations

Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment.

Caregiver 3 had been employed and working with children for 23 days before the completed national criminal background check results were returned. Caregiver4 had been employed and working with children for 4 days before the completed national criminal background check results were returned.
Plan of Correction: We will ensure to have fingerprints before employment going forward.
Standard 8VAC20-800-170-B
The provider shall obtain from each caregiver at the time of hire and prior to coming into contact with children a current TB Screening/test. The form shall have been completed within the last 30 days and be signed by a physician.

Caregiver 3 had a TB screening dated greater than 30 days prior to the date of hire.
Plan of Correction: We will work to get new TB screening done.
Standard 8VAC20-800-230-A
The provider shall maintain written documentation of each caregiver's applicable education and programmatic experience.

Four assistant caregiver files did not contain proof of education (at a minimum a high school diploma).
Plan of Correction: We will work to provide transcripts.
