During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed.

Inspector Notes
An unannounced, on-site monitoring inspection was initiated on April 24, 2025 and completed on April 24, 2025, as a part of the current licensure period. The on-site inspection began at 9:10am and ended at 10:00am. The inspector reviewed compliance in the areas listed above. There were 9 children present and 4 caregivers; the point total was 21. The inspector reviewed 1 child file and 4 caregiver/household members files on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Violations
3Caregiver 3 had been employed and working with children for 23 days before the completed national criminal background check results were returned. Caregiver4 had been employed and working with children for 4 days before the completed national criminal background check results were returned.
Caregiver 3 had a TB screening dated greater than 30 days prior to the date of hire.
Four assistant caregiver files did not contain proof of education (at a minimum a high school diploma).
