Inspector Notes

This monitoring inspection was initiated by licensing staff and concluded on 06/22/2022, between the hours of 10 am to 11:30 am. There were 6 children present (19 points), ranging from infant to preschool-age, with 3 caregivers supervising. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; special care and emergencies; and nutrition. A total of 8 children records, 1 household record, and 3 staff records were reviewed.

Information gathered during the inspection determined that there was a non-compliance with an applicable standard. This violation was documented on the Violation Notice issued to the home.

Standard 8VAC20-800-90-C
Based on review, 1 of 8 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for a child (who was not exempt from immunization requirements according to subsection B of this section) once every six months for children under two years of age.

Evidence -
1. On the date of inspection (06/22/2022), the current immunization page on file for Child #5 was dated - 08/09/2021.
2. On the date of inspection, based on information on file, Child #5 was under the age of 2 years.
Plan of Correction: Parent will be contacted to obtain an updated immunization page for Child #5.
