Violations

2
Standard 8VAC20-800-90-C
Based on review, 3 of 8 children files indicated that the Family Day Home (FDH) did not obtain documentation of additional immunizations for children (who were not exempt from the immunization requirements according to subsection B) once every six months for children under the age of two years.

Evidence -
1. On the date of inspection the following immunization pages were observed - For Child #1 (dated - 10/27/22), for Child #2 (dated - 10/25/22), and for Child #6 (dated - 11/09/22).
2. Child #1, #2, and #6 were all under the age of two years on the date of inspection.
Plan of Correction: Updated immunization pages will be obtained and placed on file.
Standard 8VAC20-800-960-E
Based on observation, not all of the required information was labeled on a bottle that was refrigerated on-site within the FDH.

Evidence -
1. On the date of inspection (05/30/23), one bottle being stored within the FDH refrigerator was labeled with the name of Child #8, but was missing today's date.
2. This was confirmed by the Provider. All other bottles were observed with both the name and date labeled on them.
Plan of Correction: Will correct.
