Inspector Notes

This monitoring inspection was initiated by licensing staff and concluded on 06/04/2024, between the hours of 9:30 am to 11:30 am. There were 8 children present (24 points), ranging from infant to preschool-age, with 4 caregivers supervising. The children were engaged in the following: tummy time, diapering, toy play, story time, music, learning, hand washing, nap time, getting ready for lunch, and other organized activities. The inspector reviewed compliance in the areas of administration; physical plant; staffing and supervision; programming; medication (see violation notice); special care and emergencies; and nutrition. A total of 9 children files, and 4 staff files were reviewed.

Information gathered during the inspection determined that there were (2) non-compliances with the applicable standards. These violations were documented on the Violation Notice issued to the home.

Standard 8VAC20-800-220-A
Based on observation and interview, the Family Day Home (FDH) does not have a caregiver who has completed the Medication Administration Training (MAT) in order to safely perform medication administration practices (listed in 8VAC20-800-710) whenever the FDH has agreed to administer prescription medications or nonprescription medications.

Evidence -
1. On the date of inspection (06/04/2024) it was determined that medication to be administered for Child #6 (on an as needed basis) was observed on site. The start date for Child #6 was listed as 06/03/2024.
2. The Provider stated that neither her nor her assistants were MAT certified.
3. This non-compliance also contradicts the signed Decision to Administer Medication form (dated - 06/03/2024) that states, "I (or other caregivers) will not administer any medication (prescription or non-prescription).
Plan of Correction: Will return medication to child's parent.
Standard 8VAC20-800-60-B
Based on review, 1 of 8 children files indicated that not all of the required information / documentation was contained within each child's record.

Evidence -
1. On the date of inspection (06/04/24), a written authorization for emergency medical care was not available for Child #2.
2. This section of the child's record form for Child #2 was left blank.
