Inspector Notes
An unannounced renewal inspection was conducted on 02/14/2023 from approximately 1:25 p.m. ? 4:10 p.m. Caregiver, household members, children?s records and the physical plant were reviewed. There were 11 children present that were between 2 years old and four years old with 2 caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. A total of 5 child records were reviewed. Caregiver and household member records were reviewed. The information gathered during the inspection determined violations with applicable standards or law. A violation notice was issued. Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from today, 03/01/2023. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). 5Evidence:
HHM1 (Household Member) was observed providing care and supervision of children during the inspection on 02/14/2023. The provider advised that HHM1 was needed to assist in the care of the eleven children in the family day home as the prior assistant left employment 01/ 27/2023. The provider did not have documentation of the orientation required by VAC20-800-200.C
Evidence:
The record for P1 contained verification of 11 hours of annual training in 2022.
Evidence:
Licensing inspector observed seven children resting in the classroom at the end of the living room. The rest mats were observed to be less than 12 inches apart.

1. The records for C1, enrolled 12/05/2022, C2, enrolled 12/05/2022, and C3, enrolled 12/13/2022 did not document the parent?s signed acknowledgement of the receipt of the information required by 8VAC20-800-70.6 and 7.

The record for C4 did not document: the name, address, and telephone number for two people to contact in case of an emergency if the parent cannot be reached (blank); the names of persons other than the custodial parents who are authorized to pick up the child (blank); the first date of attendance (blank); the parent?s signed acknowledgement of the receipt of the information required by 8VAC20-800-70.6 and 7; and authorization for emergency medical care (blank).
3. The record for C5, enrolled 02/03/2022 did not document: the addresses of the child?s parents; the work phone number of the parent, if applicable; the addresses of the two emergency contacts; the names of persons other than the custodial parents who are authorized to pick up the child (blank); the annual parent confirmation of up-to-date information in the record as required by 8VAC20-800-780.B; and the authorization for emergency medical care (blank).
