Inspector Notes

An unannounced renewal inspection was conducted on 02/14/2024 from approximately 9:00 a.m. ? 12:14 p.m. Nine children were present aged two years old to four years old with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. A total of five child records were reviewed. Caregiver and household member records were reviewed.

The information gathered during the inspection determined violations with applicable standards or law. Please sign the violation notice and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days.

Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Based on staff record review, the provider failed to obtain the results of an out of state criminal history name check, sex offender registry search and out of state child abuse and neglect search from any state in which the individual has lived in the preceding five years.

Evidence:
Staff record for P2 documented on the sworn statement having lived out-of-state in the last five years. The records for P2, employed 05/10/2023 did not contain the results of an out of state criminal history name check, a sex offender search for a state in which the individual lived in the preceding five years, and the results of the out of state child abuse and neglect search.
