Inspector Notes
An unannounced renewal inspection was conducted on 02/14/2024 from approximately 9:00 a.m. ? 12:14 p.m. Nine children were present aged two years old to four years old with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. A total of five child records were reviewed. Caregiver and household member records were reviewed.
The information gathered during the inspection determined violations with applicable standards or law. Please sign the violation notice and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days.
Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Staff record for P2 documented on the sworn statement having lived out-of-state in the last five years. The records for P2, employed 05/10/2023 did not contain the results of an out of state criminal history name check, a sex offender search for a state in which the individual lived in the preceding five years, and the results of the out of state child abuse and neglect search.
Evidence:
The immunizations for C1 were not signed by the licensed physician, the physician?s designee, or an official of a local health department.
Evidence:
The TB screenings for caregiver, P1 were dated 05/15/2021 and 07/09/2023.
The TB screenings for household member, HHM1 were dated 05/12/2021 and 09/23/2023.
The TB screening for household member, HHM2 were dated 03/23/2021 and 09/28/2023.
that shelter-in-place procedures shall be practiced twice per year.

The emergency drill documentation did not document a second practice of shelter-in-place procedures during 2023.
